1. Our Pledge Regarding Protected Health Information
MedFront LLC understands that medical information about you and your health is personal. We are committed to protecting medical information about you. We create a record of the care and services you receive at our pharmacy to provide you with quality care and to comply with legal requirements.
Under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), we are required by law to:
- Maintain the privacy and security of your Protected Health Information (PHI).
- Provide you with this Notice of Privacy Practices describing our legal duties and privacy practices regarding PHI.
- Promptly notify you if a breach occurs that may have compromised the privacy or security of your unsecured health information.
- Follow the terms of the Notice of Privacy Practices currently in effect.
2. How We May Use & Disclose Your Health Information
The following categories describe different ways that we use and disclose health information without needing your specific written authorization:
A. For Treatment
We may use your health information to dispense compounded and prescription medications, consult with your prescribing physician or nurse practitioner, screen for drug allergies and interactions, and provide counseling on proper dosage administration.
B. For Payment
We may use and disclose your health information so that the medications and services you receive from our pharmacy may be billed to and payment collected from you, an insurance company, or a third-party pharmacy benefit manager (PBM).
C. For Healthcare Operations
We may use your health information to evaluate the quality of our compounding processes, train pharmacy staff, conduct internal audits, review formulation protocols, and improve patient support systems.
D. Required by Law & Public Safety
We will disclose health information when required to do so by federal, state, or local law, including reporting controlled substance dispensations to the Texas Prescription Monitoring Program (PMP), responding to health oversight agencies, or preventing a serious, imminent threat to health or safety.
3. Uses Requiring Your Written Authorization
Other uses and disclosures of health information not covered by this notice or applicable laws will be made only with your written permission.
MedFront LLC will NEVER sell your health data or share your Protected Health Information for commercial third-party marketing purposes.
If you provide us permission to use or disclose health information about you, you may revoke that permission, in writing, at any time.
4. Your Health Information Rights Under HIPAA
You have the following rights regarding health information we maintain about you:
- check_circle Right to Inspect & Copy: You have the right to inspect and obtain an electronic or paper copy of your pharmacy records. We will provide a copy within 30 days of your request.
- check_circle Right to Request Amendments: If you believe medical information in your record is incorrect or incomplete, you may request an amendment in writing with a supporting reason.
- check_circle Right to Confidential Communications: You have the right to request that we communicate with you about medical matters in a certain way or at a certain location (for instance, by phone only, or to an alternate address).
- check_circle Right to Request Restrictions: You have the right to request a restriction on how we use or disclose health information for treatment, payment, or operations. If you pay for a prescription entirely out-of-pocket, you may ask us not to disclose that record to your health insurer.
- check_circle Right to an Accounting of Disclosures: You have the right to request an "accounting of disclosures"—a list of certain disclosures we have made of your health information outside of routine treatment, payment, and operations.
- check_circle Right to a Paper Copy: You are entitled to receive a paper copy of this notice at any time upon request.
5. Complaints & Privacy Officer Contact
If you believe your privacy rights have been violated, you may file a complaint directly with MedFront LLC or with the Secretary of the Department of Health and Human Services (HHS). We will never retaliate against you for filing a complaint.
